The Kentucky Hospital Association has partnered with I-PASS Patient Safety Institute to reduce communication errors in patient handoffs through the I-PASS Kentucky Hospital Association Transitions and Communication in Hospitals (KHATCH) program. KHA will fund KHATCH for an eight year period in an effort to establish a universal language for patent care across their member network of 128 hospitals statewide.
