The latest issue of Pediatrics features a new commentary written in collaboration with I-PASS co-founder Dr. Christopher Landrigan examines important insights from a recent study focused on care transitions within the pediatric care setting.

The study set out to design and validate a questionnaire that captured caregivers’ perspectives on the pediatric hospital admission process. The findings from the survey underscore the critical need to actively engage patients and families to ensure successful care delivery and transitions. To foster this type of family-centered care, clinicians must leverage solutions that help create a shared mental model between patients, families, and caregivers to help ensure critical information is effectively and reliably communicated.
